Situation Description: The roamer driver was heavily scolded by his boss for failing to do his job at work, and his anger soared to the head and accelerated to 100 mph on a busy highway in his 2006gavril roamer. Then I turned the steering wheel sharply on a curve, and the vehicle's ESC worked, but it went out of control and fell down a cliff.

injured: The roamer driver sustained serious injuries to his neck and other joints while the vehicle overturned. The roamer driver managed to find his cell phone and tried to make a call, but the cell phone was badly damaged and unable to make calls. He didn't get to the hospital until a few hours later another driver found the roamer driver. So his injuries got worse.

    vehicle: 2006 Gavril roamer V8 4WD LXT

Repair Cost: The vehicle was badly damaged and could not be repaired and was scrapped.

After the accident: He was discharged a few months later, and he bought the ETK 800 series, which is safer in case of an accident again, to get a little more hurt.

Situation Description: A heavy truck and a gavril D pickup truck crashed backwards.
. The heavy truck driver had to drive a long distance of more than 1000 km from the dawn of that day, so driving was so boring, and eventually he put on earphones and watched the soccer game aloud. He said in court that he didn't keep a good eye on the road because there weren't many cars on the road. When his large truck crossed the center lane, the pickup truck driver honked his horn in shock and tried to avoid it, but he was terrified and his body became frozen like ice and crashed.
What's more, the tons of weight attached to the heavy truck helped make the pickup truck even more brittle.

Injured Person: The heavy truck driver suffered minor bruises to the abdomen, and the pickup truck driver was caught in the frame and died because he had a smaller and lighter body compared to the heavy truck. Paramedics said the situation was horrendous.

Vehicle: Gavril B heavy duty truck, Gavril D series

repair cost: The heavy truck had some damage to the front weight and radiator, costing $700 to fix, and the pickup truck was scrapped because it was damaged beyond recognition.

Additional Actions After the Accident: The family of the deceased pickup truck driver said that the heavy truck driver, who had not been imprisoned, should be imprisoned, and the judge changed the decision and the heavy truck driver was imprisoned. The pickup truck driver's funeral took place a few weeks later.

Failure To Maintain Clearance, Ibishu Covet, Gavril T-Series and Gavril D-Series, Fatal accident occurred April 5th, 2021, in So Cal-Nevada Interstate, USA:

(Overview of damage to the Covet, opposite direction of travel)
    • Location: So Cal-Nevada Interstate, USA
    • Date & Time: 04/05/2021, 623 PDT
    • Vehicle(s): Ibishu Covet / Gavril T-Series / Gavril D-Series
    • Owner/Operator: Private / Southern Cargo Transports / Private
Vehicle Damage: Destroyed / Minor / Substantial
    • Defining Event: Failure To See and Avoid
    • Injuries: 1 (Fatal), 2 (Serious), 1 (None)
    • Type: Personal (Covet and D-Series), Transportation (T-Series)

The accident investigation was initially under the jurisdiction of the local police department. On April 10, 2021, the investigator in charge of the accident requested delegation of the accident investigation to the BHAI, which the BHAI accepted on April 25, 2021.

The Ibishu Covet driver reported that he was following a Gavril T-Series truck while maintaining a safe distance, at one point, both vehicles were approaching an intersection which promoted them to slow down and stop. The driver reported that, after they came to a complete stop, he observed a speeding pickup truck approaching from his right back, he exited the car, but his passenger was unable to do it in time, after jumping, he observed the pickup rear end his car, which pushed it towards the stopped T-Series truck and crushed it. All vehicles came to the rest within the accident area, the Covet came to the rest on the opposite direction of travel, sustaining extensive damage. The front seat passenger onboard the Covet was fatally injured, and the two occupants of the pickup truck were seriously injured.

The original IIC asked the BHAI to takeover the investigation because he was unable to determine if the Covet driver's story was accurate, because the pickup truck occupants reported he was not maintain(ing) enough safe distance from the T-series truck and that he (the Covet driver) abruptly stopped to brake check them, which promoted them to slam on the brakes and crash into the back of the car.

On scene examination of the accident scene after the vehicles were removed showed tire marks that were consistent with the Covet driver story, the tire marks where observed in the area between the Covet and T-Series truck (based on accident photographs), and no brake marks were noted behind the Covet's original position.

Examination of the vehicles revealed that there were no anomalies consistent with a preimpact failure or malfunctions.

Toxicology testing performed on the drivers of the Covet and D-Series at the Forensic Sciences Laboratory was negative for carbon monoxide, ethanol, and drugs.

A study of the pickup truck's cellphone records revealed that he was texting about 9 seconds before the collision. Additionally, a study of the driver's history showed he was familiar with the accident location and overall highway layouts, his history showed multiple tickets for distracted driving and failure to put on the seatbelt. According to first responders, neither the driver or the passenger in the pickup were wearing their seatbelts, which resulted in their serious injuries.

It is most likely that the pickup driver was texting before the accident, therefore he was unable to notice that the traffic ahead of him was stopping, causing the rear-end collision.

    - Probable Cause:
    • The failure of the D-Series driver to maintain clearance from stopped traffic due to his usage of his smartphone while driving. Contributing to the severity of the injuries was the failure of the D-Series occupants to wear their respective seatbelt/safety equipment.

Cheers, these stories are actually inspired by the real life NTSB (https://en.wikipedia.org/wiki/National_Transportation_Safety_Board) reports, which are basically investigation reports with in depth detail, I also have a thread for plane accidents but that one is inactive due to the lack of time and the big effort I put into the stories there :).

CASE: Police Chase Becomes Deadly (1ST SEPTEMBER, 1995)

DESCRIPTION: A 22 year old male robbed a house, and was driving away at speed. Police units soon took up the chase. Unit 12 attempted to block him in a nearby ditch, but the suspect drove straight through the hole. Even though the front bumper was hanging off and slowing him down, the suspect carried on.

Soon, Unit 11 was approaching the suspect. Unit 12 and 11 tried to block both lanes. Just as they were starting to form the roadblock, the suspect tried to overtake Unit 12, scratching the car as he did so. Unfortunately, there wasn't enough space to get through, and he slammed into Unit 11.

VEHICLES INVOLVED: 1992 Ibishu 200BX Type-LS, Bruckell LeGran Police Package (Unit 12), Bruckell LeGran Police Package (Unit 11)

INJURIES: Both Unit 11 and the suspect were killed on impact.

    LOCATION: East Coast, USA

AFTERMATH: Unit 11 was buried a few days after the incident. He was the Police Department's best driver, and the entire team struggled to cope with his death.

The suspect's body was donated to a nearby hospital, and his remaining organs were used in transplants to save other people's lives. His family were forced to pay $17,436 for property damage.

Situation Description: The driver of the ibishu covet had to go home as soon as possible due to an urgent personal situation related to work. So he accelerated to 60 mph on a cramped mountain pass and eventually fell off a cliff after losing control over a sharp curve he had never seen before.

Injury: His old ibishu covet was not up to date. So, there was only one safety belt, and when the vehicle crashed into a tree and overturned due to lack of safety, he suffered serious head and neck injuries. The D-series ambulance was too big to traverse the cramped mountain pass, so his transport to the hospital was delayed, which made his injuries worse.
The neck injury was cured with treatment, but the head injury caused brain damage and left him physically disabled.

    vehicle: ibishu covet LXI

After the Accident: His ibishu covet was damaged beyond repair and was scrapped, and his family sued the city for speeding on a mountain pass, but he would not have been seriously injured if there were guardrails installed. His family realized the dangers of an accident and bought a newer and safer vehicle than a covet.

    location: cliff road

Situation Description: The driver of the ibishu covet was a first-time visitor to the east coast. So he didn't know what the road looked like, and as the straight road continued in an alley, he thought it was a highway, accelerated to 60 mph, and then collided with a soliad wendover coming out of an alley next door. The collision was so violent that the two vehicles were pushed to the guard line on the other road.

Vehicle Damage: The ibishu covet had damaged windshield and driver's side windshield, and the A-pillar was slightly bent. Engine parts made of plastic were badly damaged.
The soliad wendover had a slight bend in the windshield and A-pillar, and damage to the engine compartment.

Injury: The driver of the ibishu covet hit his head hard on the steering wheel in the crash, but was not seriously injured and sustained bruises because he was wearing a soft hat.
The soliad wendover driver suffered a concussion because he was not wearing a seat belt and fractured his wrist bone.
Fortunately, the road was close to the hospital, police station and fire station, so the two were quickly transported to the hospital.

    vehicle: Ibishu covet, soliad wendover

    location: East Coast, USA

Aftermath: The ibishu covet driver made a full recovery, and the soliad wendover driver's family tried to file a lawsuit against the ibishu covet driver, but this was canceled because the soliad wendover driver was also at fault for not wearing a seat belt.

Situation Description: The Gavril-D driver was about to be late for his job, so he drove his Gavril D on the highway to about 100 MPH. He quickly overtaken vehicles on the road, and on the way out, he glanced at the wall in front of him. However, the heavy Gavril D had a very long stopping distance and applied the brakes, but it was already too late. He hit the guard line at about 90 MPH.

Vehicle Damage: The Gavril D was hit by a guard line at high speed, and the engine compartment was almost completely shattered, and the windshield and side glass were all shattered. The A-pillar was badly bent, and the door and rear window were damaged.

injury: The driver of the D-series suffered serious injuries to his legs and arms in the crash, and he banged his head hard against the steering wheel and died on the spot.

vehicle: Gavril D15 4WD extended cab

    location: west coast, USA

aftermath: The D-series driver's funeral was held a week later, and the vehicle had a large body frame and other accessories that were badly damaged and cost a lot of money to repair, but the bereaved family couldn't afford it, so they bought an ibishu pigeon that has space and is very cheap.

Situation Description: Gavril B heavy truck driver had to get from Nevada to his quarry in Utah as quickly as possible. So he accelerated his heavy, big Gavril B to about 80 mph. However, he saw a Gavril T truck in front of him and tried to overtake it by abruptly turning the steering wheel, but his Gavril B truck was so heavy that he could not balance left and right and fell over several times.
Fortunately, the truck overturned, avoiding all of the Autobello Piccolina in front when the wheels and the dump bed separated from the frame. Later, when investigating the accident, the autobello driver said that he was very scared at that moment.

Vehicle Damage: The Gavril B truck overturned badly at high speed, so the body frame was bent, the wheels and the dump bed were separated from the body, and the front, side and rear windshields were all broken. And the roof of the cab was dented.

injury: Fortunately, the truck driver was wearing a seat belt when the vehicle was overturning, so no serious injuries were reported. He even crawled out of the truck and called 911 himself. He suffered injuries to his neck and back. However, the truck driver was located in the middle of a very long highway, so it took a long time for the ambulance to arrive. So the Autobello driver behind the truck put a splint on the truck driver and gave him painkillers until an ambulance arrived to make the pain less painful.

    vehicle: Gavril B75 mining, semi

    location: Nevada, USA

aftermath: The truck driver was fully healed and discharged from the hospital a few weeks later, and met the Autobello driver to express his gratitude and gifts for helping him during the accident.
The Gavril truck was badly damaged and scrapped, and the truck driver's company said it would replace the truck with a newer one for the safety of the drivers.
