    Case: Loss of control, 5th July 2005

    Description: It was 10.45 AM. During the trackday, a driver of a very limited ETK i-Series lost control and hit the wall twice.
    The car was made in 50 stock, and his car was the first destroyed one. He didn't hit anybody, so noone else was

    Injuries: The driver twisted his ankle in his left leg.

    Vehicles: 1991 ETK i-Series DTM Edition /w 3.0L i6 engine (special config I made)

    Location: Hirochi Raceway, USA

    Aftermath: Since there was no spare parts for the DTM version, the car couldn't be fixed. ETK fined the driver 10,000$ for damages

    Case: failed overtake, March 29th 2021.

    Description: an impatient driver tried to pass a slower car while approaching their exit on a roundabout. They had to cut across and caught the front of the other car, losing control and hitting a truck that was waiting to merge. The other car parked up on the exit so they could exchange insurance details, but the impatient driver tried to move as well and ended up reversing into them.

    Injuries: none.

    Vehicles involved: 2020 Cherrier Tograc 110d, 2004 Bitron B2, 2008 Gavril B85.

    Location: Scottish Highlands.

    Aftermath: the Bitron driver was extremely shaken, but his car was fine and continued in service with him. The Cherrier driver was found fully at fault but received no punishment.

    "Drunk Dodging"

    Case: Drunk Driving leads to two separate accidents, June 6th, 2004

    Description: After finishing his shift at Turboburger, Derek Samson, 18, was returning home from work with a his friends/roomates, when Patrick Browne, a 28 year old news reporter, was heading towards Mr. Samson's 200BX, slowly drifting into his lane. Mr. Samson laid on his horn and swerved at the last moment to avoid a head on collision, causing him to crash his heavily modified 200BX into a tree. Mr. Browne then swerved back into his lane and carried on driving. Luckily, Maria Pearce, a 34 year old police officer, was heading in the same direction as Mr. Samson and his friends, and after making sure everyone was ok and making her partner, 27 year old Jacob Moore, stay with the teenagers and tend to Mr. Samson's wounds with the med-kit in the trunk of her cruiser while calling in the accident on her radio, turned around and chased down Mr. Browne's car until she found it, crashed into a second tree a few blocks away.

    Injuries: Mr. Samson had a mild concussion and was dazed for a few minutes after the accident. Mr. Browne was surprisingly uninjured.

    Vehicles involved: Heavily modified 1993 Ibishu 200BX Type-LS, 1999 Ibishu Pessima 2.0 LX.

    Location: Garfield Heights, Ohio, USA.

    Aftermath: For the entire time Mr. Samson was tended to by paramedics, he was more concerned about his car then himself, and after getting out of the hospital the next day, had his car towed to a local body shop, which fully repaired his car in around a month. Mr. Browne was sent to the hospital and checked over by doctors to make sure he was ok, after which, he was arrested for DUI, reckless driving, and public endangerment, and was sent to jail for a year and had his license suspended for two. Mr. Browne's Pessima was totaled, and was sent to a junkyard where it was parted out and eventually crushed a few years later.

    Case: reverse impact, November 1st 2010.

    Description: a worker on a construction site headed out to get food for his fellow workers on their lunch break. While he was ordering at a cafe, another car parked behind the truck he had taken to get there. When reversing out of the place, he hit the car and pushed it back a few metres without even realising until he was reprimanded by its owner.

    Injuries: none.

    Vehicles involved: 2008 Gavril B85, 2002 ARZ 200.

    Location: Scottish Highlands.

    Aftermath: the car's owner was found at fault as he had parked in a bad place. Due to the lack of damage to the truck, its driver received no punishment from his employer.

    Case: Cluch pedal stuck, 23 October 2015

    Description: A young boy was speeding from his parents house to his house with one friend. The gas pedal got stuck and he crashed and flew for a few seconds.

    Injuries: The boy and the friend were unharmed, but were transported to the hospital for a check.

    Car(s): 1988 Soliad Wendover, 1991 Ibishu Covet, 1998 Gavril D15 Farm

    Location: Fishing village, East Coast, USA

    Aftermath: The Wendover and the Covet were totaled. The people in the Covet, two Bosnians, were shocked and they got back to Bosnia 2 days later.

    Case: rear-end collision, January 4th 2009.

    Description: a Gavril driver, upon realising he was being overtaken by an Ibishu, sped up to try and block it out. The driver of the Ibishu car therefore started to move over when they thought they had passed the slower vehicle, however the two cars collided and the Gavril swerved onto the hard shoulder- where a broken-down car sat. It hit the car from behind at about 80km/h, sending it soaring across the live lanes where it was hit again, though at lower speed, by the Ibishu. Both the Gavril and the broken-down car burst into flames upon impact.

    Injuries: the Gavril driver and the two people in the back of the broken-down car were all killed in the fire. In trying to rescue them, the broken-down car's two other occupants (who had gotten out before the crash) suffered serious burns.

    Vehicles involved: 2006 Ibishu Pessima DTi wagon, 2008 Gavril Vertex S, 1993 Charmand 1104B.

    Location: Italy.

    Aftermath: the Gavril driver's family claimed it was a fault with his car that started the fire, however the Charmand's owner said he had broken down due to overheating which would have been a more likely cause. The Ibishu driver was fined for careless driving.

    Case: failed overtake, March 2nd 2021.

    Description: a semi truck overtaking another truck on a blind bend met a car head-on. Swerving back across the road, the semi truck driver swiped the front corner of the truck he was passing with his trailer and rolled his truck over.

    Injuries: the two occupants of the oncoming car were critically injured, with the driver suffering a major head wound. He unfortunately passed away during emergency surgery that same day. The semi truck driver had a broken arm and leg, and concussion.

    Vehicles involved: 2009 Gavril T65 (semi), 1985 Gavril T75 (tipper), 2006 Gavril Roamer.

    Location: East Coast, USA.

    Aftermath: the SUV's passenger survived, though her injuries were life-changing and medical bills left her in a lot of debt. The semi truck driver was fired, not only for dangerous driving but also for breaking the truck- it would not drive after the crash and had to be towed away. The SUV was also totalled.

    • Defining Event: Failure To See and Avoid
    • Injuries: 3 (Fatal), 1 (Serious)
    • Type: Transportation (both)


    - Analysis:
    The 41-year-old flatbed truck driver reported that he was ordered to arrive on one of the airport gates along with a co-worker in their vehicle, call sign "Van (2)", according to the driver, it was extremely dark at the time and the runway lights were not turned on due to maintenance works, the runway was closed for aircraft and all arriving flights were diverted to the nearby airbase, located a few killometers from the accident airport.

    The driver reported that as he was crossing the runway intersection, he saw bright lights approach from the right, and a collision occurred immediately. The driver, his passenger, and the two people onboard the second vehicle were not wearing seatbelts, which resulted in fatal injuries to the second van's passenger and the two people onboard the first van.

    According to eyewitnesses, it seemed that neither driver was aware of the other's location, as they were approaching the runway intersection at a high speed without using any source of lights (front lights... etc).

    Three other drivers who were parked in their vehicles (which was using the same radio along with the accident vehicles) reported that the radio seemed silent around midnight, the drivers only remembered one of the two accident occupants reporting that they started their engine and that they were headed to Gate 1, so they would use the runway intersection, no more recordings were heard from either accident drivers up to the collision. A study of phone and radio records confirmed the eyewitness accounts.

    A study of both drivers records showed that one of them (Van (1) driver) had several violations and tickets throughout his history, two of them included failure to properly communicate while on duty while driving on an active runway, one of them resulted in an airplane preforming a go-around at the last few seconds. The second driver had a similar recorded violation but it was determined that it was not serious.

    Examination of the vehicles revealed that there were no anomalies consistent with a preimpact failure or malfunctions. Additionally, toxicology testing performed on the drivers at the Forensic Sciences Laboratory was negative for carbon monoxide, ethanol, and drugs.

    It is most likely that both drivers were not aware of each other's presence on the active runway because they failed to report their positions, speeds, and approximate location on the radio. Additionally, both vehicles were not visible for each other because neither of them had any light sources turned on, which deemed the collision unavoidable. A contributing factor is that the Air Traffic Control Tower was busy informing arriving flights to divert to a nearby airport due to the emergency landing of an airplane on the runway, which damaged several runway lights and signs.

    - Probable Cause:

    • The failure of each driver to report/confirm their approximate location while approaching an active runway intersection without any light sources, which resulted in a T-bone collision. Contributing to the accident was the ATC's diverted attention due to the closure of the runway for aircraft due to an emergency landing that damaged several runway equipment. Also contributing to the severity of the injuries was the failure of all occupants involved in the accident to wear their respective seatbelt/safety equipment, and the lack of safety oversight by the Airport Authority.


    - General Information:
    On September 20th, 2020, at around 504 pacific daylight time, a Gavril H45 Diesel Cabster Flatbed Upfit, callsign "Van 2", was crossing an runway intersection when it was hit by a vehicle of the same type, callsign "Van 1" at the Los Injurus Metropolitan Airport, Los Injurus, USA. The certified driver and passenger onboard Van 1 and the passenger onboard Van 2 were fatally injured, and the Van 2 driver was seriously injured. Both vehicles sustained substantial damage as a result of the collision. Both vehicles were traveling on the airport runways that were closed due an airplane making an emergency landing and causing several delays and diverts. Nighttime visual meteorological conditions prevailed at the time of the accident, and there were no prior emergencies reported by any of the vehicles involved in the collision.

    According to the surviving driver, he was called by the boss to move the vehicle to Gate 1 for inspections, a co-worker decided to join him on the estimated "1 minute trip", which involved crossing a runway intersection. A study of the "Van 1" planned route suggested the driver and his backup driver were traveling down the runway, do a 180 and continue to the main terminal area. (Figure 1)

    Additionally, the driver reported that neither him or the second driver were using the radio to report their positions, which resulted in the accident, he added that extreme darkness prevailed and neither vehicle had any light sources turned on, rendering the accident unavoidable.

    Eyewitnesses reported an identical scenario as described the driver, both vehicles, despite being not visible to them, were heard approaching the intersection at "a rather high speed". A collision occurred and all of the occupants were not wearing their seatbelts or safety equipment. Three people were declared deceased on the scene upon the arrival of first responders.

    The H45 Cabster has a 0.5m longer wheelbase, heavy duty frame, heavy duty suspension, and heavy duty wheels. In addition, it can be fitted with various upfits, including a cargo box or flatbed. The Flatbed variant is powered by a 6.0L Diesel V8 engine and weighs around 3150kg. The vehicle has a top speed 106 mph.

    Examination of the accident scene revealed that both vehicles came to the rest on an identical heading, with substantial impact damage noted on both (Figure 3). Further on scene examination of the vehicles revealed that there were no anomalies consistent with a preimpact failure or malfunctions. The vehicles was transported to a secure facility for further examination.

    in black hills town the bruckell monhawk are going stright down the road then charger driver want to go right and he is a bit on road but then the monhawk driver crash into charger fender next driver of moonhawk lost control and crash into parked roamer then driver of coronet lost control too and smash his back of the car into bulding

    Case: road rage incident, April 28th 2021.

    Description: an ETK driver was cut up by a taxi, and was so furious that he turned around and tailed the taxi for over a mile. After two unsuccessful overtaking attempts and being brake-checked twice by the taxi, plus a lot of swearing and gesturing on the ETK driver and passenger's parts, the ETK tried another pass on the taxi. However, this time the taxi sped up in an attempt to block the raging ETK out- the result was that when the ETK moved back across the road, it caught the front end of the taxi and was spun around.

    Injuries: the fight that ensued saw the taxi's window smashed with a hammer, leaving its driver with a lot of glass cuts. The ETK's passenger, who had been the one with the hammer, received a black eye from the taxi's passenger who attempted to break up the fight, as did an oncoming driver who stopped to help. There were no injuries from the crash itself.

    Vehicles involved: 2020 Cherrier Vivace 110d, 2015 ETK 854td.

    Location: Italy.

    Aftermath: the ETK's two occupants were charged with assault, as was the taxi passenger. The ETK driver was found at fault, though the taxi driver still had his licence suspended.

    Case: Distracted driving incedent

    Description: A Cherrier driver was using the CADS(Cherrier Auto Drive System) on the highway to the city.A sunburst driver was stopped at a red light at the same time.The cherrier driver was on his phone while the car drove itself.He lloked up and saw he was about to hit the sunburst.He swerved bu hit both the sunburst and the traffic pole.

    Injuries: The sunburst driver had whiplash but was fine otherwise.The cherrier driver suffered a broken arm.

    Vehicles involved: 2020 Cherrier Emousse E,2014 Hirochi Sunburst

    Location: WCUSA Highway i95

    Aftermath: The cherrier driver payed out for the damages with no trying to claim it was the sunburst drivers fault.They 2 later became friends.

    Case: loss of control, October 26th 2003.

    Description: a street racer spun his car's wheels up at speed, spinning across the road and crashing over some rocks.

    Injuries: the driver escaped with a few cuts, concussion and a broken leg.

    Vehicles involved: 1993 Ibishu 200BX.

    Location: Utah.

    Aftermath: the car was a complete wreck. No legal action was brought against the driver due to only one witness, his opponent, refusing to give evidence in court.
