    - Collision : Gavril T-Series Semi Trailer Truck, owned and operated by the driver, and Dansworth D2500 (Type-D) Rear Engine Coach Bus, owned and operated by Public Desert Transportation Co., Fatal accident occurred January 14, 2018, near the Desert Region Airport, in the Desert Roads Area, USA.

(Computer generated photos of the crash done during the Investigation)
    • Date: 14 / January / 2018
    • Site: near the Desert Region Airport, in the Desert Roads Area, USA.
    • Vehicle(s): Gavril T-Series Semi Trailer Truck and Dansworth D2500 (Type-D) Rear Engine Coach Bus
    • Injures: 1 (Fatal), 1 (none)
    • Damage: Truck: Minor / Bus: Substantial (Damaged beyond repair)
    • Summary: Collision

    Preliminary Report:
On January 14th, 2018, a Gavril T-Series Semi Truck Trailer, owned and operated by the driver, was headed to a nearby power planet when it collided head on with a public transportation Dansworth D2500 (Type-D) Rear Engine Coach Bus near the Desert Region Airport, in the Desert Roads Area, USA. The Coach was owned and operated by Public Desert Transportation Co., of Reno, Nevada.
The sole driver on board the truck was not injured and his vehicle sustained minor damage. The sole driver on board the coach bus was fatally injured and his vehicle sustained substantial damage. Weather conditions at the time of the accident were described as a "sunny clear day perfect for trips".
The truck driver reported that while he was traveling at the narrow road, the bus continued in his direction, and despite trying to avoid the collision, the bus struck his trailer which caused it to detach.
The NATSB sent a Highway Accident Investigator to examine both vehicles and determine the probable cause.

    Damage:
    • Gavril T-Series:

The Gavril T-Series truck was transporting an empty trailer container to a nearby power planet. The trailer separated from the main truck cab due to the collision.
The cab sustained very minor damage to the right headlight and tire when the driver attempted to maintain clearance from the incoming bus.

    • Dansworth D2500 (Type-D) Rear Engine Coach Bus

The 25-seat coach bus was traveling to a local bus station to transport passengers around the area. The bus has been operating for a few years, and there were no previous incidents with it.
The driver of the bus was described as "experienced and careful". A study of his logbook revealed he had logged more than 6000 driving hours in his career and that he wasn't involved in any incidents where he was at fault.

A post crash examination revealed that the bus struck the truck's trailer with the left side (the driver's side), the driver immediately applied the brakes and the bus came to a stop approx 5 seconds after the collision.
The coach's far left corner was crushed towards the back by the impact, the door and 6 left side passenger windows were damaged, the steering wheel and instruments were crushed towards the driver's seat, the cover behind the driver's seat almost crushed the first passenger seat to the left. The entrance way to the front door was crushed by the impact. Both headlights, the bumper, the front grille, the windshield, both mirrors, and marker lights were either destroyed or substantially damaged by the impact. The front left wheel's rubber was pealed from the rim.

    Analysis:
The driver of the truck reported that he was driving below the speed limit in a narrow road, he was transporting an empty trailer to a nearby power planet. While driving in the narrow road, he saw the coach bus approaching from the other lane, he said that the bus "was going on a straight direction", but the current part of the road required turns since both vehicles were approaching different curves. The truck driver said the bus didn't correct its direction which forced him to stick to the safety wall, the truck driver recalled avoiding the bus before hearing an impact sound.
The driver looked in the mirror and he instantly knew that the trailer struck the bus. He called 911 and they arrived in approx 2 minutes.
The truck's cab sustained very minor damage to the left front side. The coach bus sustained substantial damage to the entire front section.
The driver of the coach bus survived the initial collision, but he later passed away due to the injures sustained in the collision.
The truck driver reported that there were no pre-accident mechanical failures or malfunctions with the truck that would have precluded normal operation.
Examination of the coach bus also revealed there were no preaccident mechanical failures or malfunctions that would have precluded normal operation.

    - Probable Cause and Findings:

The Highway Accident Investigation determines the probable cause(s) of this accident to be:

    • The bus driver failure to maintain clearance from an upcoming truck/trailer in a narrow road, which resulted in a side by side collision between the bus and the trailer.

    - Loss of Control : Off-Road Gavril Roamer, owned and operated by the driver, Fatal accident occurred March 19th, 2019, in Crimson Valley, USA.

    (Final resting position)
    • Date: 19 / March / 2019
    • Site: Crimson Valley, USA
    • Vehicle(s): Off-Road Gavril Roamer
    • Injures: 3 (Fatal), 1 (serious)
    • Damage: Destroyed
    • Summary: Loss of Control

    - Preliminary Report:
On March 19th, 2019, an Off-Road Gavril Roamer was traveling in the Crimson Valley Area when it departed the lane and experienced several role overs until coming to the rest inverted. The vehicle was destroyed. The driver, front seat passenger, and the back seat left passenger were fatally injured. The back right seat passenger was seriously injured. The vehicle was owned by the driver and operating for him at the time of the accident. Weather conditions around the time of the incident were described as "clear and sunny".
The NATSB sent a junior investigator to assist the Crimson Valley Police Department and determine the probable cause of the occurrence.

    - Damage:

    (Final Resting Position)
The roamer came to the rest inverted. The back left section of the car was crushed downwards due to the rollovers, both taillights were destroyed, and the back door/window departed the structure. The front section was also crushed downwards by the several impacts, the engine and the surrounding parts were damaged. Both front doors opened inadvertently due to the crash. All windows were broken, and despite wearing seat belts, the three occupants that were occupying seats near the downward crushing forces were fatally injured. The surviving occupant was wearing a helmet, which contributed to his survivability.

    - Analysis:
The roamer was traveling at about 100 mph according to the CVPD. The vehicle came to the rest upside down near an intersection, a few meters away from the original departing point, which was an immediate curve after an uphill, a quick test revealed that this part of the road is challenging and should be approached at low speed.
A check on the driver's logbook revealed that he had a history of reckless driving in the Crimson Valley area, he once had a similar incident with another 4x4 car, he was the sole occupant onboard and survived with minor injures. The investigation from that incident reveled that the driver's recklessness and failure to follow the speed as the cause.
The medical examiner determined that the three fatalities were due to multiple blunt force trauma due to a car crash. The surviving passenger said he couldn't recall the events of the day.
Examination of the vehicle also revealed there were no preaccident mechanical failures or malfunctions that would have precluded normal operation.

    - Probable Cause and Findings:

The Highway Accident Investigation determines the probable cause(s) of this accident to be:

    • The driver's failure to follow the recommended speed in a curvy road that lacked necessary visual reference despite his experience and knowledge of the area, which resulted in the vehicle's departure from the road, and subsequent rollovers.
    • Contributing to the survivability of the right back seated passenger was his usage of a safety helmet

I will try to keep posting in this thread before I get busy with my HS year.

Trusty? More like Faulty!: Part 1 of 3.
Description: Belasco City outskirts, June 3rd, 2019. A 16 year old student driver, by the name of Tom Aniya, was driving with his instructor, Daniel Miles, on the Matrix Freeway outside of Belasco City for Tom's final test. At one point a cop was following him and he forgot to indicate when he was changing lanes, causing 40 year old sheriff, AJ Dashera, to pull him over. He apologized and AJ went to run Daniel's ID (the instructor is responsible for what the student does) and ran the student driver vehicles plate. Meanwhile, 56 year old Mike Sampson was driving his old "trusty" Gavril T-series semi carrying steel pipes on a flat-bed trailer. He was being followed by Kelley Ryes, who was late for work. The highway was pretty much empty since it was after morning rush hour was over. Mike noticed the steering on his semi was a bit stiff at times and a bit loose, causing him to not be able to steer. He and Kelley were approaching the traffic stop when suddenly his steering failed and his semi veered over into the path of AJ's cop car. luckily AJ was out of the car talking to Tom and Daniel, and was able to jump out of the way. The semi rammed the cop car, pushing it into the student driver vehicle, causing the student driver vehicle to be spun around and rolled into the wall. The cop car went down and under the semi and was sandwiched between the truck and the left freeway shoulder. Kelley pulled over to help. Daniel called 911 and first responders were there in under 3 minutes luckily, because there wasn't much traffic.
Injuries: AJ was a little scraped up from jumping over the concrete median barrier and landed on the other side of the road. Tom had a bruise on his hand from hitting the side of the door. Daniel was a bit shaken up. Mike was shaken up as well. Kelley was also a bit shaken up from seeing it unfold.

    Map used: Matrix Freeway

    Cars used: 2 1995 Gavril Grand Marshall cop cars, 1 1997 Gavril Roamer, 1 1992 Gavril T-series, 1 2014 Hirochi Sunburst, 1 1997 Gavril H-series ambulance, 1 2017 Hirochi SBR4

Lawsuit/Aftermath: The company Mike drives for sued Gavril for the price of the truck as the truck was faulty but Gavril unfortunately won using the excuse, "the truck is old, we can't help that it aged." The truck, SBR4, and cop car were totaled, each car having some kind of bad damage. since then, only 11 days since the crash, 17 other reports of faulty T-series steering have been reported. Tom ended up getting his drivers license and Mike is planning to retire when he turns 60. AJ had to take a medical leave because it turns out he had dislocated his shoulder and had to have surgery. Kelley was promoted a few days later.

Hey guys, I haven't posted in a while as well, and to celebrate this threads "resurrection", I thought I'd post 2 stories.

You're license is a myth too.

April 3rd, 1997. There was a small Model Train convention going on in the town of Atlanto. Lots of people from all over the Island were crowding into the small town, which caused several jams on some of the major roads. Dylan Sims was one of the people heading to the convention. In front of him was Chris Hooker in his Roamer, Cara Dukes in her Covet, the Acker family in their Pessima, and the Crawford family in their Kashira. As the small group of cars were driving, Dylan noticed something off the coast. The Myth of the Abandoned Ferry. He slowed down to 47mph and tried reaching for his phone to take a picture. He was so incredibly focused on getting his "proof" that he didn't notice that the group had stopped at an intersection so the Crawfords could turn left. Dylan rear ended Chris, which launched it into Cara, which bumped into the Ackers. Kathlyn, who was heading the opposite direction, tried avoiding Chris' Roamer. She wasn't able to get out of the way in time and collided with the front of the Roamer.

Injuries: Dylan had a broken left wrist, fractured right arm, broken left ankle and knee, and some minor head injuries. Chris had severe whiplash and a broken wrist. Cara also had a little bit of whiplash. The Ackers had no injuries. Kathlyn had a bruised left knee and a sprained wrist.

    Map used: Atlanto

Cars used: A Mercedes-Benz C Class, a Gavril Roamer, an Ibishu Covet, an Ibishu Pessima (2nd Gen), an Ibishu Kashira, and an Ibishu Pessima (1st Gen).

Aftermath: Dylan tried to claim that Chris rear ended the line first and he wasn't able to react fast enough, but the CCTV from a nearby Gas Station proved that statement wrong. Dylan had to pay fro the cost of all 5 cars, including medical bills. Dylan's car and Chris' car were deemed to be totaled.

    Time for School.

March 23rd, 2009. Ralph Moore was taking his 2 kids, Flynn Moore (13) and Sienna Moore (15) to school after a doctors appointment for both of them. Ralph has recently been diagnosed with a minor Seizure Disorder and was instructed to take a SPECIFIC kind of medication. Just before the drive, he had accidentally taken vitamins instead of his medication. As he was driving, he started to have a seizure. His van veered from the left lane to the shoulder several times before he finally crashed into a barrier, which through his van into the oncoming lane. In the opposite direction, Journey Arnolds in his Mazda had just overtaken Kim Stiles, who was driving her bus route. Journey just barley manged to avoid the van while Kim stopped just in time. Anthony Hodson, who was traveling behind Ralph and is an EMT, pulled over and took care of Ralph until emergency services came.

Injuries: Ralph has a broken right wrist, broken left knee, broken right ankle, and a cut on his head. Flynn and Sienna had only bruises since they were at the very back of the van.

    Map used: Atlanto

    Cars used: An Ibishu Kashira

Aftermath: Ralph was taken to the hospital and his wife came to pick up the kids. Ralph recovered from his injuries but still has a seizure disorder, but now he has a service doggo!

Trusty? More Like Faulty!: Part 2 of 3
Description: Belasco countly line, June 14, 2019. Joseph Andrazo was driving his Gavril T-series flatbed to a port to pick-up some crates to deliver down south to good ol' texas. He was flooring it as he wasn't going very fast down the freeway, and realized when he let up, the pedal didn't go up, and when he tried to turn his steering wheel to swerve and slow the truck down, he realized his steering was stuck, too. A sheriff of belasco, Kandice Smith was driving behind Miriam Willis and her two kids, Dave (8) and Susan (14), when Kandice noticed the truck barrelling down the road, she was too late to warn a merging car from being clipped by the out of control truck. She saw the truck not stopping after hitting the vehicle, so she got behind it and turned her lights and sirens on, warning other drivers to move to the far left-or-right lanes. A few miles up the road, Jamar Dosiah was stuck in his twoima (not sure if I spelled that right) after running over tire pieces from a truck blow-out, and wasn't able to get out because he didn't want to get hit by any speeding cars. The truck was still gaining speed. A man coming home from a family get together, Jackson Davison, recorded the truck from his pick-up. The truck, with many witnesses to see it, plowed into the Twoima without warning, spinning it into the wall, driving the truck into the wall as well. The twoima violently hit the truck and flipped, and the truck hit another part of the wall, sandwiching the twoima. Kandice and the wtinesses all quickly stopped to help. Miriam called 911 and another officer, an ambulance, and a paramedic car were on scene within a few minutes. Joseph got himself out of the car and was helped to sit down by a few witnesses, while other witnesses, the officers, and the ambulance crew worked to get Jamar out of the car. Eventually, they were able to pull jamar out of the car and laid him down on the truck's bed.

Injuries: Jamar had multiple bones broken in both legs and feet, and had at least 6 cracked ribs, and some head trauma, along with a few other serious injuries. Joseph had a broken ankle and a few bruises.

Map used: Matrix freeway (again xd)

Cars used: 1995 Gavril Grand Marshal (Alternate), 2 1995 Gavril Grand Marshal PI's (Police cruisers), 1997 Gavril H-series ambulance, 1994 Ibishu Covet, 1995 Ibishu Twoima (still not sure if I spelled that right xd), 1996 Gavril D-series, 1998 Ibishu Kashira, 1994 Gavril T-series, 1997 Gavril Roamer, 2017 ETK 800 series

Lawsuit/Aftermath: Gavril was sued once again for the faulty T-series, and this time, Gavril lost the lawsuit and had to pay up over 250k for the cars, damages, medical costs, and the other accidents that occurred involving faulty t-series. Jamar made a full recovery and is now a volunteer paramedic. Joseph quit his job and is now a mechanic.

not alot of structure there, doesn't look like there were 5 deaths, also please use maps with a actual environment and not simple maps like this.

Added more accurate description of deaths for blind people like you. And if you didnt know, a crash doesnt need to look fatal to be fatal.

Crash Description: Chris Taylor had just bought a brand new Orange GT500 and decided to test it out on the freeway during rush hour. While driving he encountered some unexpected traffic. Instead of slowing down he decided It would be a good idea to speed up and try to weave through traffic. He quickly lost control as he was not used to his brand new 700hp Mustang. He swerved to miss a Roamer and rear-ended a Grey ETK 800 Sedan. They both spun out and caused the cars behind them to slam on the brakes, which then caused them to be rear ended by the next cars in line. When the whole thing cleared up, 4 cars had been totaled including the Kashira which had been flipped, and 4 more had suffered minor damage.

Injuries: The driver and passenger of the Kashira and the driver of the Jeep had minor whiplash and Chris suffered a broken arm. Other than that everyone was okay with just minor scrapes and bruises.

    Map Used: SSRB

Cars Used: Ibishu Kashira, Ford Mustang, 1994 Covet

Aftermath: The Kashira, Jeep, ETK 800 and the Mustang were Totaled. Chris was charged with speeding and reckless driving.

    • Vehicle(s): ETK 800 / Ibishu Covet
    • Injures: 3 (Fatal), 1 (serious)
    • Damage: Destroyed (Written off)
    • Summary: Collision
    - Preliminary Info:
On February 23rd, 2019, at about 0754 eastern daylight time, an ETK 800 vehicle, was occupied with a driver and traveling to a pickup location located about 40 miles from a local airport in the Desert Roads Area when it struck an Ibishu Covet that traveling in the opposite lane. As a result of the accident, the three occupants onboard the Covet received fatal injures, and the driver of the ETK 800 received serious injures. The Covet broke up into two, debris was scattered all over the impact location, the debris also followed the path of the ETK 800, which came to a rest in a downhill a few meters from the original impact location. Both vehicles were transported for a private location for further examination.
The ETK 800 vehicle belonged to a private traveling agency which uses an internet app to book trips and VIP transportation of the app followers.
A truck driver who witnessed the accident said that "The ETK was traveling at a very high speed before crashing into the Covet", "Engine parts, wheels, and other stuff were scattered all over the place!".

    - Analysis:
According to the ETK driver, he was traveling to pickup a family of four from a local restaurant, the driver reported that despite the clear weather that day, he preferred to relay on the GPS and available instruments to conduct the trip. The driver reported that he saw the other vehicle approaching a few seconds before the impact, but it was too late to stop. Examination of the ETK vehicle recorded data revealed that the brakes were applied approx one second after the initial impact, the ETK kept traveling at the same speed before the impact.
The driver of the ETK 800 stated that there were no pre-accident mechanical failures or malfunctions with the truck that would have precluded normal operation.
Examination of the Ibishu Covet also revealed there were no preaccident mechanical failures or malfunctions that would have precluded normal operation.

    - Damage :
The high speed impact occurred when the ETK 800 veered to the other lane, striking the front left of the Covet (driver side), the impact forces and crushing damage caused the Covet to break into two parts, the ETK came to a full stop about 8 seconds after the initial collision.

The ETK 800 sustained major damage to the entire front section, the front left wheel separated from the vehicle, the engine sustained crushing damage, and the entire front coverage was destroyed by the impact forces. There was no post crash fire.

Wreckage and Impact Information [ETK 800]:

    Driver Injuries: 1 (Serious)
    Vehicle Damage: Written off
    Passenger Injuries: N/A
    Vehicle Fire: N/A
    Other Injuries: 3 (Fatal)
    Vehicle Explosion: N/A
    Total Injuries: 1 (Serious)

The four seat Ibishu Covet broke into two due to the high speed collision, the engine was ejected from the compartment, the entire front section was crushed, the front/back headlights and bumpers detached due to the impact forces. The four seats were removed from the scene by the Highway Emergency Department, but first responders accounts revealed that they were ejected to the road.
The interior parts, some engine parts, the four doors, and other parts were scattered in a way that followed the path of the incoming ETK 800. The back of the Covet itself rotated about +90 degrees.
Examination of the dashboard instruments were ineffective due to the high level of destruction.

Wreckage and Impact Information [Ibishu Covet]

    Driver Injuries: 1 (Fatal)
    Vehicle Damage: Destroyed (Written off)
    Passenger Injuries: 2 (Fatal)
    Vehicle Fire: N/A
    Other Injuries: 1 ( Serious)
    Vehicle Explosion: N/A
    Total Injuries: 3 (Fatal)

    - Probable Cause and Findings:

The Highway Accident Investigation determines the probable cause(s) of this accident to be:

    • The ETK 800 driver over reliance on the GPS system, which resulted in his failure to see and avoid an approaching vehicle, subsequently causing the collision. A Contributing factor was the failure of the ETK driver to arrest his speed.

Trusty? More Like Faulty!: Part 3 of 3
June 30th, 2019, WC, USA, state line. A road service driver, Neil Decatom, 55, has been called out to the state line on highway 156 to clear debris off the road from a previous accident. He got there within 55 minutes as the department was quit a ways away from this location. Mean while, Asher Hampton, 29, was driving down the highway when he went to go down the hill at the location. When he saw the road service vehicle, he decided he wanted to help so he slowed and put his emergency flashers on on his covet. 3 miles back on the highway, Jason Arez, 65, is driving his gavril T-series dump truck out of state to get rocks from a quarry to bring back to belasco. Eventually, he gets to the hill and spots the two vehicles. He tries slowing down but, he realizes his brakes are not working and his steering is tightening up. He starts barrelling down the hill, causing motorists to panic and swerve into the median or into the right lane. He tries to avoid the two stopped cars but can't and plows into the covet, pushing it into the flatbed where it is launched into neil who is picking up pieces in front of the flatbed. The flatbed then rolls into the median as the truck flips on its side. The covet is crushed but luckily Asher is not in the vehicle and jumps out of the way just in time to avoid getting hit by the flat bed. Several motorists stopped and called 911. One call goes as follows:

Dispatcher: Hello 911, state the nature of your emergency please.

Caller (Amorita Sancha): Yes I just witnessed a bad accident on highway 156!

Dispatcher: Ok what part of highway 156?

    Caller: The state line!

Dispatcher: O-ok.. uhm.. west bound or eastbound ma'am?

    Caller: Eastbound..

Dispatcher: And is there any injuries?

Caller: I don't know... But two people were out of their vehicles and one got hit!

Dispatcher: Ok honey stay on the line for us ok?

    Caller: Ok!

The caller gave details about the crash and talked to the dispatcher about what happened until emergency units arrived. It is a good thing people stopped and called 911, or this crash could have become much worse.

Injuries: Jason has some scratches and bruises and a few other minor injuries. Neil was killed when his flatbed rolled over top of him. Asher was a bit scratched up from landing on the median.

    Map used: Classic Autobahn

    Cars used: 1994 Ibishu Covet, 1998 Gavril T-series, 1996 Gavril H-series.

Lawsuit/Aftermath: Gavril finally makes a Re-call on the T-series model line-up. Amorita and Asher ironicly became a couple. Jason made a full recovery and still works for the trucking company. Total damage was worth over $100k USD.

May 8, 1974. 1973 Moonhawk V8 Special with two occupants (driver and front passenger) hit a 1971 Autobello Piccolina with just a driver head on. Two people survived, you can probably guess who.
