Crash Description: A asleep truck driver was sitting in the middle of the road while the truck was just sitting there running because the truck driver forgot to turn the truck off. Then, a distracted 2012 grand marshal driver crashes into the back of the truck, being killed instantly. then, an ETK k-series convertible then collides into the back of the GM, also being killed instantly, then a daewon coach bus crashes into the back of the GM, which resulted in 3 people getting injured. Then a couple of cars come from the opposite direction and cause accidents. Then, a kei truck imported from japan also comes from that lane, colliding into the truck, being killed instantly. Then, a Gavril vertex collides with the kei truck. The vertex driver survives too. The pile-up eventually stops but with injuries and deaths. Case: Fatal crash in Texas, USA, July 13, 2023, 16:20. Description: The accident occurred when the driver of the RV was not paying attention when turning left, turned in front of the Ferrari, which hit the left side of the RV, causing the RV to spin into the opposite lane, where an Opel crashed into it, while the driver of the Dodge behind the Ferrari was distracted by the accident, not paying attention to what was in front of him, causing him to crash into the Shelby turning right, which spun from the collision and came to a stop in the middle of the road, the Nissan then drove into the back of the RV and came to a stop on the side of the road, and finally the Mercedes-Benz truck slammed head-on into the RV, the impact lifting the front of the truck into the air, causing the truck to crash into the front of the Shelby in the middle of the road. The accident was captured by the on-board camera of a GMC driving behind the Nissan, which pulled over at the scene of the accident and then its driver called for help. The accident was also filmed by a passenger in a broken-down bus on the side of the road and the bus driver (the bus driver was waiting behind the bus for the tow truck and filming passing cars when the accident occurred). January 23rd, 2014 Crash description: Joe Hartman (Age 47) was speeding down the road and driving recklessly as usual but he was distracted due to not having his contact lenses. He crashed into James Hollingsworth (Age 58) who was driving a Soliad Lansdale going to his 121 year old grandmothers house to take care of her for the day. They collided head-on which sent the Tahoe into a rollover before coming to a stop. James and Joe both went to the hospital for x-rays and check-ups. Injuries (if any): Joe had a broken leg, 1 broken rib, and some other broken bones and injuries. James only had a broken wrist on the left. Crash Description: A very rich italian mafia leader was strolling around when he noticed a Gavril Grand Marshal which was one of their targets. He chased the grand marshal before he pit maneuvered it into a guardrail. Both vehicles were heavily damaged and could not be repaired. Both vehicles were also pretty expensive. Injuries (if any): Glass shards, broken arm and broken foot for the grand marshal driver, meanwhile the mafia leader suffered a mild concussion and some other injuries he preferred to not disclose due to personal reasons. July 18th, 2016 It was a warm evening in Fairhaven, USA. A man named Terrence Lawson was driving with his family at high speed in their SUV. All of the sudden, a car pulled out in the road in front of them. This car held Lina Bruckell (Driver), Sammy Gutan, Alice Armanson, and Catrina Lawe (passengers), who were all drunk. The SUV slammed into the side of the car at high speeds. Injuries: Lina and Sammy died about a minute after the accident. Terrence broke his arm and a glass shard got stuck in his hand from the other car. His son Billy got whiplash. Everyone else suffered minor injuries. Cars: GMC Yukon GMT800, Dodge Magnum. Fatal Passenger Van Collision With a Tourist Motorcoach and Subsequent Rollover, Utah, USA. (view of both vehicles where they came to rest) What Happened On July 6, 2022, at about 1051 local time, a Gavril H-Series, operated by BeamCon UT Adventures, was traveling down a dirt road when it approached an intersection with a yield sign. At the same time, a Dansworth D2500 Type-D rear engine Coach Bus, operated by Tourism Transporting LLC, was approaching the intersection while traveling down a single lane road without any markings when the Van entered the intersection. The motorcoach impacted the right side of the van. The van rotated twice around its axis before coming to a stop. The motorcoach veered to the right and down an embankment into a dirt road, rolling over once before coming to rest on its right side. There was no post crash fire. As a result of the accident, five van passengers and six motorcoach passengers sustained fatal injuries. The drivers of both vehicles sustained serious injuries. The remaining four van passengers and fifteen coach passengers sustained injuries ranging from minor to serious. What was found The probable cause of the accident was determined to be: The failure of the van driver to yield to incoming traffic, which resulted in an intersection collision with a motorcoach due to fatigue (lack of sleep). Factors relating to the accident were BeamCon UT Adventures inadequate training procedures and failure to monitor their drivers sleep history. Contributing to the severity of the motorcoach occupant injuries were the failure of the coach driver to conduct a pre-trip safety briefing as described in the operator's safety manual, which resulted in multiple passengers not wearing their seatbelts and subsequent ejection during the rollover sequence. Also casual to the accident was the speed of the motorcoach as it approached the intersection. As a result of this accident investigation, several safety recommendations were published. Factual Narrative On July 6, 2022, at about 0951 local time, a Gavril H-Series, operated by BeamCon UT Adventures, and a Dansworth D2500 Type-D rear engine Coach Bus, operated by Tourism Transporting LLC, were both substantially damaged when they were involved in a collision in Utah. USA. five van passengers and six motorcoach passengers sustained fatal injuries. The drivers of both vehicles sustained serious injuries. The remaining four van passengers and fifteen coach passengers sustained injuries ranging from minor to serious. Daylight visual meteorological conditions prevailed at the time of the accident. According to the local Police Department, in the van driver's post accident interview, he stated that he was approaching the intersection at low speed and entered it without any problems, 1 second later, he heard a loud bang and blacked-out before waking up in the hospital. According to the driver of the motorcoach, he was traveling down the road when the van "appeared out of nowhere" infront of him. The driver stated that he had no time to react or control the bus after the collision. According to the van operator, the purpose of the trip was to take nine tourists on a local trip to visit multiple tourist attractions and spots. The operator stated that the van driver was dispatched to pick up the passengers from a local hotel at about 0525 in the morning. The operator stated that the driver informed them about reaching the hotel at about 0540. The trip reportedly started at 0550. Cellphone records and passenger interviews showed that the van stopped at three different locations. The reported accident trip destination was a local hiking camp. According to the motorcoach operator, the purpose of the trip was to take 21 out-of-state visitors to a local river attraction before proceeding to a nearby landmark. The driver picked up the passengers from a hotel in Las Vegas, Nevada, at about 0711 in the morning. According to several motorcoach passengers, the driver appeared to be "in a hurry" and forgot to the before start safety briefing. One passenger, who is familiar with the operator, stated that one of their policies is for the driver to give a five-minute safety briefing before the start of every trip to explain the importance of safety belts and how to exit the coach in case of an emergency. A review of the operator's safety manuals showed that the passenger's statement was consistent with what was written down in the manuals. Surviving motorcoach passengers stated that the driver told them about "taking a shortcut" through a valley with narrow roads. Several passengers reported that the driver was speeding down a few tight corners which was "unusual". - Injuries: As a result of the accident, five van passengers and six motorcoach passengers sustained fatal injuries. The drivers of both vehicles sustained serious injuries. The remaining four van passengers and fifteen coach passengers sustained injuries ranging from minor to serious. According to the County medical examiner, all five of the van passengers died from significant head trauma and multiple blunt force trauma. They were seated in the right section of the vehicle. The deceased motorcoach passengers were either ejected or partially ejected from the vehicle. Several other passengers were displaced and "thrown" around in the passenger compartment. - Highway Information: On the collision location, the road consists of one travel lane at each side, which are not separated by any markings. The area of the collision does not have any light sources (light poles... etc). The intersection has a yield sign (Figure 1) for incoming traffic from the dirt roads. There is also an "unimproved road" sign at the entry of the right side dirt road. (Figure 1: Accident Intersection and Signs) - Vehicles: (1) Motorcoach: The bus was originally built as a school bus. According to the manufacturer company, A “type D school bus” is a body installed upon a chassis, with the engine mounted in the front, midship or rear, with a gross vehicle weight rating of more than 10,000, designed for carrying more than ten persons. The engine may be behind the windshield and beside the driver’s seat; it may be at the rear of the bus, behind the rear wheels, or midship between the front and rear axles. The entrance door is ahead of the front wheels. A type D school bus has a maximum length of 45 feet, and can carry up to 24 passengers. Maintenance records pertaining to the accident bus, obtained from the operator, included approximately 3 month of daily vehicle inspection reports (DVIR), as well as 3 years (March 2019—March 2022) of maintenance and inspection records. No major mechanical system defects of the accident bus were noted in the DVIRs provided. The accident bus’s maintenance records noted a variety of regularly scheduled maintenance and as-needed repairs. (2) Van: The Gavril H15 Van is a passenger van made for carrying more passengers. It is equipped with an independent front suspension, a live axle rear suspension, and a 4.5-liter V8 engine. There were maintenance records recovered during the course of the investigation. - Examination: A post accident examination of major mechanical systems for both vehicles revealed there were no pre-impact failures or malfunctions that would have precluded normal operations. - Drivers: (1) Van: The 35-year-old driver possessed a Utah class license with a passenger endorsement. The license was issued in 2021. A search in the driver's history found no revocations or suspensions for this driver. His most recent medical examination for CDL fitness determination was completed in January 2021; it was valid for 1 year from issuance. The shorter-than-usual expiration was due to the driver’s hypertension and sleep apnea (for which he was being treated). During treatment at the hospital, blood and urine were collected for diagnostic purposes. Toxicological screening of the blood for alcohol was negative, and the urine screening was negative for specific drug substances. (2) Motorcoach: The 60-year-old driver possessed a Utah class license with a passenger and truck endorsements. The license was issued in 2019. A search in the driver's history found two speeding violations in the last five years. Test results were negative for alcohol and all other drugs for which he was tested. - Records: According to the van driver's company records, there were no mention of his sleep apnea issues. The driver reported to the investigative team that he told the operator about these issues multiple times, but the operator failed to take a notice of this. According to logbook history, the van driver was dispatched seven times in the day before the accident. The driver completed his last trip at about 0200 in the morning of the accident trip. The driver reported that he slept for about 3 hours before he got a call from his operator to pickup the passengers. A review of company training procedures for intersection instructions showed that the drivers were taught to "proceed at their own caution" when they think it is safe to do so. The van driver reported that it was casual for him to proceed to intersections if he didn't "feel" any incoming traffic approaching him from the other sides. Review of the motorcoach operating procedures showed that the drivers were instructed to follow the posted speedlimits for any road, but the manuals did not contain any info on how to handle narrow roads with no posted speedlimit signs. The driver of the motorcoach reported that he did not see any speed restrictions in the accident location and "felt" it was okay to go at a "higher than typical" speeds at this section. - Damage: The motorcoach initially struck the right side of the van. Damage began at the van's front side passenger door and extended midway towards the rear. The van's right side body was bowed inward from impact. The right side main passenger door was impact damaged and separated. The right side front passenger door was impact damaged and partially attached to the frame (Figure 2). The van seats remained mostly intact and were slightly displaced towards the left (Figure 3) (Figures 2 and 3) The motorcoach sustained impact damage to the front side (in both directions) from the initial collision (Figure 4). The bus continued down an embankment and sustained structural deformations at several spots due to the rollover sequence. Several side windows were destroyed. No passenger seats were displaced. Some passengers were able to exit via the left side emergency door while others were extracted by emergency personnel (Figure 5). NOW TODAY, June 30, 1933 74 year old man killed after reckless driving incident leads to ejection of driver. The body was found about 115 feet away from the accident scene. Autopsies revealed that the man was under the influence and went off road to overtake a corner. He lost control which eventually lead to ejection. The body was very beat up with major organ damage and about 11 ribs fractures. Arm, Skull fractures were also present which was the reason the man died. Police also found some illegal stuff in the car, which was later confiscated. Crash Description: EMS were cleaning up a crash scene involving a Ram 1500 and a gavril T-series twin-steer cement truck. A Cadillac Eldorado then drove up to the scene and then a Gavril MD-series box truck also drove up to the crash scene. The bridge then collapsed due to both the weight and its age (It wasn't Maintained since 1996). Then, some vehicles came plummeting towards the ground, with more to follow. All the vehicles fell down. Future EMS got there when they were notified. Injuries (if any): Gavril MD-series driver had a broken leg and severe tissue damage on the thigh thankfully. gavril truck driver died instantly. Dodge Ram driver had serious injuries and had to lifeflighted to a hospital. Gavril Grand Marshal driver died on the way to the hospital. Cadillac Eldorado driver died in the hospital overnight. 2 Paramedics died when they were doing some things and the bridge collapsed and 3 were injured heavily. Map Used: Utah USA.
