    Hospitals aren’t gonna pay 120k+ for an impractical ambulance.

    Well, if you take a basic spec E-Class and just put an E63 drivetrain in, it might even work.

    I think even a E45 drivetrain would work
    350 hp in a ambulance

    This I can see happening, since the E43’s drivetrain is, in all fairness, a hotted-up E400.

    would you prefer a 380 hp 3 liter v6 or a 375 hp 2 liter 4 cylinder turbo?

    There's also the matter of running costs, and maintenance. Worst scenario to happen if something breaks, it's going to be alot more expensive than it is on what's currently being used.

    V6, although a better choice for that level of power would be a V8. Easier to drive, better throttle response, cheaper to maintain.

    Yeah. Not necessarily a V8 but a smaller V6 or a 4 banger.
